preoperative medical consultations; hence, reducing unnecessary consultations among low-risk patients while potentially increasing consultations among high-risk individuals. Notably, the existing variability in preoperative consultation practice may actually be helpful in designing initiatives to improve the appropriateness of preoperative medical consultations. Specifically, in characterizing preoperative consultation practices at centers with superior postoperative outcomes, we may be able to identify specific practices that can be implemented more widely at other centers. As suggested in the accompanying editorial, improved prospective databases will be critical for any such effort to use existing practice variation for identifying “optimal” perioperative practices. Once there is better understanding of which patients should be appropriately referred for preoperative medical consultation, anesthesiologists must take a leadership role in developing preoperative evaluation processes that better target appropriate patients for such specialized preoperative care. The goal for these improved processes should be to simultaneously improve clinical outcomes and patient satisfaction, while reducing healthcare system costs and practice variability.

Predictors of pain relieving response to sympathetic blockade in complex regional pain syndrome type 1.

BACKGROUND Sympathetic blockade with local anesthetics is used frequently in the management of complex regional pain syndrome type 1(CRPS-1), with variable degrees of success in pain relief. The current study investigated which signs or symptoms of CRPS-1 could be predictive of outcome. The incidence of side effects and complications of sympathetic blockade also were determined prospectively. ...

Complex regional pain syndrome (CRPS) type-1 following snake bite: a case report.

The pathophysiological mechanism and clinical course of complex regional pain syndrome (CRPS) type-I still remain ill defined. Both the treatment and the prediction of the outcome of the treatment are difficult. Abnormal neurohumoral and inflammatory mechanisms have been implicated in its causation usually following trivial noxious event in an extremity. However, to the best of our knowledge CR...

S INCE the American Civil War, clinicians and neuroscientists have been mystified by patterns of persistent pain and cutaneous hypersensitivity after injuries to the limbs that are accompanied by remarkable neurovascular, sudomotor, motor, and trophic changes. These syndromes traditionally were labeled as reflex sympathetic dystrophy or causalgia, according to the absence or presence of identif...

Reflex Sympathetic Dystrophy Complex Regional Pain Syndrome (CRPS) Recognition and Management for the Physician

Reflex Sympathetic Dystrophy is a dysfunctional pain response to a noxious stimulus. It is characterized by pain out of proportion to the inciting event, which may be quite insignificant but can be severe. In its earliest stages reflex sympathetic dystrophy is responsive to sympathetic blockade, pharmacologic agents, other modalities, and physical therapy. Early recognition and treatment is cri...

Successive multisite peripheral nerve catheters for treatment of complex regional pain syndrome type I.

Complex regional pain syndrome (CRPS) type I is a painful and disabling syndrome that is accompanied by physical changes in the affected extremity. It generally occurs after trauma, manifesting as pain that is out of proportion to the inciting event. Treatment of the disorder is difficult, with many patients being refractory to multiple pharmacologic regimens. Regional anesthetic techniques, in...
